- 38% of sites saw profits decline in 2023, and 37% have under $1 million in gross revenue (SCRS).
- About 82% of sites reported operating costs rising 6% or more in 2024, over half saw double digit increases, salaries were the main driver for about 69%, and 41.2% said they could not offset the increase (Bourne Partners, citing SCRS).

- Screen failures: 88% say they are not sufficiently covered (SCRS), and 42% say screen failure terms never or rarely cover actual screen fails (SCRS).
- Recruitment: 53% of sites regularly pay for it from their own funds (SCRS).
- Studies that never fill: in a typical trial 11% of sites enroll zero patients and 37% under enroll (Tufts CSDD). Startup work is spent either way; revenue follows patients.
- Unbilled work: prescreening, extra visits, and amendments that never make it onto an invoice.
What margins do healthy sites aim for?
Buyers of research sites treat EBITDA margins of 25% to 50% as a sign of operational discipline, according to an investment bank that advises site sales (Objective IBV). Treat that as a buyer's benchmark, not a typical result.
